REGISTRATION

Please fill out your chapter registration form and send $10 per person registration fee (payable to “UNTHSC”) to Brad Lancaster – 8138 Julie Ave. Fort Worth, TX 76116 - no later than October 3^rd, 2003.  The $10 registration fee will RSVP each student for the Friday evening wine and hors d’oeuvres reception and Saturday breakfast, lunch and keynote dinner. 

Please indicate if attendees are AMA members, AMPAC members, and/or would prefer a vegetarian option for meals.  If attendees of your chapter will be flying into DFW International Airport or Dallas Love Field, please include flight information for volunteer student transportation service.
