Faculty & Staff
Definition: Anyone verified by the Office of Human Resources as holding a faculty or staff appointment at UNT Health Science Center.
Fee: $10 per person per month
Documents required for sign-up: None
How Fees are Paid: Via UNTHSC Payroll Deduction or 1 year in full (cash or money order). If choosing payroll deduction, employees commit to at least 3 months of membership before termination can be accepted.
Faculty, Staff, and Student Spouses**, Significant Others**, and Dependents*
Definition: Individuals wishing to obtain membership based on a relationship using this eligibility must have proof that he or she shares the same address with the employee or student.
Fee: $10 per person per month; One-time $10 ID badge fee applies*.
Documents required for sign-up: Proof of address that matches the employee or student they claim relationship with and a current drivers license. Employee/student must be with member during sign-up.
How Fees are Paid: Fees are paid on a month-to-month basis via cash or money order only; Checks are not accepted. Dues must be paid by the 6th of each month to keep membership active for the month.

Alumni
Definition: Anyone verified with the Office of the Registrar has having received a degree from UNT Health Science Center.
Fee: $10 per person per month; One-time $10 ID badge fee applies* as old UNTSHC student ID badges are no longer valid after graduation.
Documents required for sign-up: Current drivers license.
How Fees are Paid: Fees are paid on a month-to-month basis via cash or money order only; Checks are not accepted. Dues must be paid by the 6th of each month to keep membership active for the month.
Faculty/Staff Retirees
Definition: Anyone verified by the Office of Human Resources as having retired from UNT Health Science Center.
Fee: $10 per person per month; One-time $10 ID badge fee applies*

Corporate
Definition: Anyone verified as working at Amon Carter Museum, National Horse Cutting Association, Fort Worth Museum of Science and History, and Modern Art Museum of Fort Worth. Membership must be approved by Fitness Center Manager.
Fee: $20 per person per month; One-time $10 ID badge fee applies*.
Documents required for sign-up: Business card or employee ID badge and current driver's license.
How Fees are Paid: Fees are paid on a month-to-month basis via cash or money order only; Checks are not accepted. Dues must be paid by the 6th of each month to keep membership active for the month.
Community
Definition: Individuals, ages 16yrs or older, who are otherwise eligible for membership and have no affiliation with UNT Health Science center through academic, employee, or student standings.
Fee: $30 per person per month; One-time $10 ID badge fee applies*.
Documents required for sign-up: Current driver's license.
How Fees are Paid: Fees are paid on a month-to-month basis via cash or money order only; Checks are not accepted. Dues must be paid by the 6th of each month to keep membership active for the month.
*All Founders' Activity Center members are required to have an ID Badge. The $10 ID Badge fee is a one-time charge, unless the ID Badge is lost. All replacement badges are an additional $10 per replacement badge.
