All new membership forms must be turned in to the Fitness Center Manager between 8:00am-5:00pm, Monday-Friday. If this time frame does not work for you, please e-mail Megan St. Vigne for more information.

FAC Usage Waiver (for ALL membership types)

 ***No one under the age of 16 is allowed in the Founders' Activity Center under any circumstances***

 (Includes use of outdoor basketball court)

UNTHSC Child Policy

FAC Payroll Deduction Form (for UNTHSC employees)
(Also used to terminate membership)
