Clinic Hours
All of our family practice clinics are open from 8 a.m. to 5 p.m. Monday through Friday. After these hours, phone calls are answered by our answering service. In an emergency, the answering service will put patients in touch with a doctor.
Appointments and Billing Questions
You may make appointments either by telephone or in person. Please tell the appointment clerk the nature of your problem or need so that she can schedule the proper amount of time for your visit. As a part of the UNT Health Science Center clinic system, osteopathic medical students, residents, physician assistants, and nurse practitioners participate on our health care team, adding an extra level of personal care for you in our clinic.
We make every effort to keep our appointments on schedule. Please help us by being on time or by calling as far in advance as possible if you must cancel an appointment.
If you have questions regarding your statement please call the billing office at 817-547-9599.
If you receive a bill from a reference laboratory that you feel is in error, please contact the reference laboratory or fax the bill to your clinic for further assistance.
Insurance Information
We participate with most HMO's, PPO's, and commercial insurance plans. We also accept Medicaid, Medicare, and Tricare. Other major insurance plans that we accept is Blue Cross/Blue Shield, Aetna , and United Health. Please check with your insurance company to see if we are on your plan. As a courtesy, we file claims on behalf of the patient. Our Tax ID # is 75-6064033.
Please be aware of your insurance plan's rules regarding treatment and payment. Many plans require a co-payment by the patient, which will be your obligation to pay at the time of service. You are personally responsible for the payment of services provided to you unless you are covered by Medicaid or specific contract plans. Having insurance does not release you of this responsibility.
If we must send you to a specialist and a referral is required, we will obtain this for you. If you make a follow-up appointment to the specialist, it is your responsibility to notify us that another referral is needed. We require 7 working days to obtain the follow-up referral. If you have any questions about your referral, you can email your referral specialist at this address - FamMedReferrals@hsc.unt.edu.
Services
We specialize in total health care for the entire family. Our family practice clinic has the experienced staff and modern equipment necessary to perform a full range of services including laboratory tests, osteopathic manipulative therapy, and minor surgical procedures.
We also offer international travel medicine and psychological services including psychotherapy, weight management, stress management, smoking cessation, and a memory clinic. Some of our clinics offer OMT, Vaccinations for Children Program, Well Women and Well Men exams, Sports Medicine, and patient education, please contract your clinic to see if they provide these services.
Veterans Affairs has established a community-based outpatient clinic at the health science center which is also administered by the Central Clinic. To sign up with the CBOC, please call 1-800-924-8387, ext. 36676.
When you need to refill a prescription from one of the clinic providers, please contract your pharmacy and inform them to fax a prescription refill to you clinic.
All medical records maintained at our clinic are the property of the clinic.
Many times it is helpful for our providers to have records or medical tests from other providers you have seen. We will ask you to sign a written request for these records, and we will mail it to your previous provider. If you want copies of your medical records or need copies sent to another physician, you must sign a request to release your record or send us a signed, dated authorization letter and provide the address for mailing.
