Immunization Form

Please complete and return the required Immunization form to the UNTHSC Student Health Services, 3500 Camp Bowie Blvd., Fort Worth, TX 76107 prior to orientation. You will need your EUID to register for classes at new student orientation.

Your EUID is the login that is required to access our student information system. It was emailed to you during the admissions process. If you have not activated your EUID, go to > the following URL https://ams.unt.edu/acctreq.php and follow the instructions. If you have activated your account and cannot remember your EUID or password, use the above URL to retrieve this information. If you have not activated your account and cannot locate your EUID/PIN, please contact our office at gsbs@hsc.unt.edu for this information. Your EUID must be activated prior to your arrival at new student orientation.

Tuition/Fees (International or Out of State)

Tuition/Fee payment deadline is May 23rd, 2008 (via credit card, check, money order or cash accepted). You may want to bring a separate bank draft to cover your tuition and fees until you can get your bank account established: Your bank draft should be made payable to 'The University of North Texas Health Science Center' or UNTHSC to cover the tuition cost for enrollment.

Course Registration: During orientation, you will meet with the graduate advisor from your respective department for academic advising. The faculty member will be advising you on courses to register for this summer semester. Medical Sciences majors will be administratively enrolled prior to orientation.
