New Student Orientation Fall 2008 (mandatory)

We are excited about your decision to attend the UNT Health Science Center-School of Public Health. Here is some important information regarding New Student Orientation.

Fall Orientation 2008 (mandatory)
Date: Wednesday, August 20th (International Students Only)

International Orientation Agenda

Time: 8:30 a.m.-2:30 p.m.

Place: EAD 706
Date: Thursday, August 21st (All Students must attend)

SPH Orientation Agenda
Time: 7:30am-5:00pm
Place: Kive Lobby/Everett Hall
Dress: Business Casual

You will need your EUID to register for classes at new student orientation.
Your EUID is the login that is required to access our student information system. It was emailed to you during the admissions process. If you have not activated your EUID, go to > the following URL https://ams.unt.edu/activate.php and follow the instructions. If you have activated your account and cannot remember your EUID or password, use the above URL to retrieve this information. If you have not activated your account and cannot locate your EUID/PIN, please contact our office at sph@hsc.unt.edu for this information. Your EUID must be activated prior to your arrival at School of Public Health new student orientation.
Please check your "To do list", to make sure we have received all necessary documentation.

Items needed before New Student Orientation:

Immunization Form
Attached below is an immunization form that needs to be completed and mailed to the our office: UNTHSC-School of Public Health, Office of Student and Academic Services, 3500 Camp Bowie Blvd., Fort Worth, TX 76107. For general information about Student Health Services at UNTHSC, go to http://www.hsc.unt.edu/education/studenthealth/forms.htm click on School of Public Health option. If you have questions about specific immunizations, please contact the Student Health Office, jhernand@hsc.unt.edu. Missing immunizations will be administered on Friday, August 22 from 11:30-1:00 p.m. in CBH 110 (Center for BioHealth) and will require a follow-up visit on Monday, August 25 in CBH 110 from 11:30-1:00 p.m. (please plan accordingly)

Student Insurance Requirement http://www.hsc.unt.edu/education/studenthealth/insurance.htm ,see downloadable required document. You will have an opportunity to submit this form to the Office of Student Affairs during orientation. Student Injury and Sickness Insurance Plan available at https://www.uhcsr.com/Ship/SHIPHome.aspx

Classes Begin:Monday, August 25, 2008

Course Registration:You will meet with a faculty member from your respective department. The faculty member will be advising you on courses to register for this fall semester. Two weeks after orientation you will be assigned an academic advisor that will help you complete your degree plan prior to the spring 2009 class registration period.

Finally, we realize we've given you a lot of details, but we want to provide you with as much information as possible so you can plan accordingly.
